Ta'awwudh & Tasmiyahالاِسْتِعَاذَة والتَّسْمِيَة

The ta'awwudh is said in the first rak'ah, before Al-Fatiha. According to al-Qurtubi, Abu Hanifa and al-Shafi'i regarded the recitation of the whole prayer as one recitation, so it is not repeated in each rak'ah. Quran 16:98 gives the command to seek refuge. The Prophet ﷺ gave the wording 'A'udhu billahi mina sh-shaytan ir-rajim' (Ṣaḥīḥ al-Bukhārī 6115; Abū Dāwūd 4781), and al-Qurtubi says it is the wording most scholars use. On the Basmala, al-Qurtubi reports that Malik held it is not a verse of Al-Fatiha, al-Shafi'i that it is, and Abu Hanifa and Ahmad, among others, that it is recited silently with Al-Fatiha.
